Muggenthaler Ästhetik sets new standards in the post-treatment of phenol peelings.

In the current issue of the Journal of Aesthetic Surgery, I present my new method of phenol peeling post-treatment to specialists and colleagues. It is fair to say that this is a milestone in the field of deep peeling – both in terms of healing and treatment efficiency. More on this in the following article.

Phenol peeling is one of the most effective procedures for facial rejuvenation and wrinkle correction. No other procedure smoothes even very deep wrinkles comparatively effectively and leads to a fascinating tightening of the entire facial skin. As good as the rejuvenating effect is, the treatment is complex and time-consuming. Up to now, the cost of immediate aftercare for phenol peeling has been particularly high, which has deterred many doctors and patients. However, special attention must also be paid to wound healing in the first week until new epithelialization is complete. Preferably in a clinic.

Together with my team, I have developed a procedure in recent years that considerably simplifies aftercare. It essentially consists of applying a mask mixed from bismuth gallate and blood plasma, which is applied immediately after peeling. This completely new procedure considerably simplifies follow-up treatment for both the patient and the doctor.

Phenol peeling – from preparation to post-treatment.

The phenol peeling treatment in our clinic is based on a 3-pillar principle: preparation, treatment and aftercare. Every single step must be carried out meticulously and professionally in order to make the treatment as comfortable as possible for the patient and to achieve the best possible result.

Preparation & skin conditioning
I consider a comprehensive skin conditioning program to be fundamental for successful skin rejuvenation. This essentially consists of the following steps: cleansing, skin protection with antioxidants, reduction of skin pigments and stimulation with retinoids. The pre-treatment is usually carried out 4-6 weeks before the actual peeling.

Treatment
The procedure itself is performed under local anesthesia and in the presence of an anesthesiologist. For phenol peeling, we only use certified ready-to-use products from Skintech. These contain phenol, croton oil and TCA in varying concentrations. I tailor the intensity of the peeling precisely to the needs of your facial skin.

Aftercare
After the phenol peel, the treated areas of facial skin are covered with a waterproof and airtight tape bandage, which only has openings for the eyes, nose and mouth. After 24 hours, the tape bandage is removed and the areas that require a deeper treatment can be exfoliated again. To facilitate the application of the mask, we prepare a paste from bismuth gallate powder and blood plasma. Prior to this, approx. 40 ml of blood is taken and centrifuged. The plasma obtained is then mixed with bismuth gallate powder until a paste-like mass is obtained, which can then be easily applied to the exfoliated areas of skin with a brush.

This new procedure is far more user-friendly than simply “dusting” the open wounds.

On the 8th day after the treatment, the mask is carefully loosened with Vaseline. Afterwards, the treated facial skin is still clearly reddened and sensitive. The skin must be treated with special ointments for approx. 3 weeks. As soon as the redness has subsided, conventional skin care, including the use of retinoids, can be resumed.

The advantages of optimized aftercare

We have found that the use of the powder/plasma mixture has significantly improved patient acceptance. At the same time, however, we also have the impression that the extent and length of the reddening of the skin has decreased significantly after the peelings and that the effectiveness of the treatment in terms of correcting wrinkles has also increased.

I am convinced that this optimized post-treatment will significantly influence and drive the further development of peelings.
